Facilitate structured dialysis starts - if possible at home

Although close to 50% of late-stage CKD patients who receive access to structured education choose dialysis treatment at home as their preference, actual numbers remain low. In case renal replacement therapy becomes necessary, we are there to support Kidney+ participants emotionally, when they and their nephrologist evaluate which treatment modality best fits their individual situation. We provide patients with validated answers, knowledge, and access to certified peers for experience exchange.

Which individuals can enrol in Kidney+?
Generally speaking, Kidney+ is currently limited to patients with a CKD diagnosis of stages 3a and above, but not yet on renal replacement therapy (e.g., dialysis or kidney transplant). They need to own a smartphone or tablet with internet access and be capable of using basic app functions such as chat and videocalling.
How do you identify program participants?
Currently, there are several ways: Patients can ask for details to enrol in Kidney+ at their nephrology practice, health insurances can team up with us to encourage specific members to enrol, and patients can also reach out to us directly to validate their eligibility in Kidney+.
Who exactly is the Kidney+ care team?
Our Kidney+ care team consists of experienced CKD nurses, seasoned dieticians and veteran patients, who have been trained to be peers for others. All coaches are generally hired by the Kidney+ program and run through extensive coaching training.
How does Kidney+ handle patient data?
Each patient owns their personal health data. When they enrol in Kidney+, they allow our care team to view some of their app data to better understand general health and program progress. Kidney+ does not share sensible health data with any third parties, unless users have explicitly given consent to do so.
Who pays for Kidney+?
Depending on the country, costs for program participation can be covered by kidney care providers, health insurances and/or affiliate partners. Patients are not being charged for any services in Kidney+.

Kidney+ is a team of caregivers, dieticians, doctors, product designers and software engineers working at the tech startup Carealytix that specializes in nephrology. Our mission is to empower people with chronic kidney disease to better access health education and resources to live a healthier life. To realize this mission, we partner with nephrology care providers, health insurances and industry partners.
