Kidney+ is a hybrid care service that empowers patients with chronic kidney disease (CKD) to best manage their health between their regular doctor's visits, with the aim of slowing down its progression and reducing dialysis crashes and hospitalizations.
Not all CKD patients progress towards later stages. In those who do, costs and disease burden rise with each subsequent stage before skyrocketing with the start of dialysis. In partnering with payers, Kidney+ offers a health coaching program especially intended for those at risk of kidney failure. All with the goal of keeping your patients healthy, out of the hospital, and off dialysis until it's absolutely necessary.
Unplanned dialysis starts are not only frequent and costly but also more likely to result in in-center treatment - both heavily impact the life of patients. The Kidney+ team accompanies enrolled participants in a personal yet scalable way. In doing so, it aims to provide assistance to better navigate transition phases in one's health journey, ensure structured dialysis treament starts and keep your insured members at risk out of the hosital.
Although close to 50% of late-stage CKD patientsⓘ who receive access to structured education choose dialysis treatment at home as their preference, actual numbers remain low. In case renal replacement therapy becomes necessary, we are there to support Kidney+ participants emotionally, when they and their nephrologist evaluate which treatment modality best fits their individual situation. We provide patients with validated answers, knowledge, and access to certified peers for experience exchange.
