The time between doctor’s visits is often long and may feel challenging. We provide you with a care team and the most advanced resources that align with your personal goals and needs. Your Kidney+ care team of nurses, dieticians, and peers is here to support you to live your best life with kidney disease. We aim to keep you healthy, out of the hospital and delay renal failure as long as possibe.
Everybody has a different journey. Together, we find the right path for you to contain your disease progression and live your best possible daily life. Your Kidney+ team helps you form healthier habits and make informed choices about your kidney health by offering validated answers to questions about diet, activity, and co. We can further connect you to peers – we know that some things are best discussed with others who have gone through the same experience.
Our goal is to keep you away from dialysis until your treating medical team decides it is necessary. However, if your kidneys’ performance is deteriorating, you and your nephrologist will work closely to choose the renal replacement therapy that best suits your everyday life and personal needs when it’s time. Your Kidney+ care team will be there to support you emotionally as needed. Whether you decide for treatment in-center or in the comfort of your home, for a kidney transplant or for any other option – you’ll have our full support every step of the way.
Download the Mizu app from the Google Play or App Store. All you need is a smartphone with internet access.
To get started, scan the QR code you received in your nephrology practice to activate the Kidney+ module in Mizu. If you haven't already, you will be asked to also create a Mizu account.
Once your app is set up, we'll guide you in scheduling your first video call with your Kidney+ coach. The aim of this meeting is to talk about the first steps of your health coaching. We look forward to accompany you on your health journey!
